WAIVER OF LIABILITY

The following definitions apply to this waiver of liability form:
“the School” means the appropriate company, Progressive Discover-e Inc., and includes its owners, shareholders and directors;
“administrator” means a person employed by the school, who oversees program development and/or holds a supervisory position in the school;
“teacher” means a person employed or contracted by the school, who holds a valid certificate of qualification to teach in schools in Saskatchewan;
“teacher aide” means a parent, guardian or appointed person of significant interest overseeing, at all times, the education of one or more children within the same family;
“other participants” means any adult or child present during any educational activity that is organized by the School but not enrolled in the School. This would include but is not limited to special presenters and family and friends of pupils; and
“pupil” means a person who is enrolled in the School, and includes the Student.

In consideration of (name of student you are registering online) (“the Student”) being permitted to participate and attend at Progressive Discover-e o/a Flex ED Registered Independent School (“the School”), I agree to save harmless and release Progressive Discover-e Registered Independent School, its Administrators, Teachers, Teacher Aides, Pupils, and any other Participants, of the school, and other agents, service representatives, and/or employees, from and against any and all claims, causes of action, costs, expenses, demands of any nature and/or damages sustained and/or caused to the student in respect to death, injury, loss or damage to person or property, howsoever caused, arising out of or in connection with (“the Student”) taking part in any activities at the school or organized by the school, and notwithstanding that the same may have been contributed to or occasioned by the negligence of School, its Administrators, Teachers, Teacher Aides, Pupils, and any other Participants, of the school, and other agents, service representatives, and/or employees.

I further agree to save harmless and release the School, its Administrators, Teachers and Teacher Aides, on behalf of myself and on behalf of the Student, from any and all claims, causes of action, costs, expenses, demands of any nature, and or damages sustained, for alleged malpractice against the school, an Administrator, a Teacher or Teacher Aide, based upon any allegations regarding the teaching methodologies, curriculum, or other activities of the School.

I further acknowledge that transportation will be required to and from all extension activities for the Student, and release the School, and its Administrators, Teachers, and Teacher Aides, Pupils and all of their Participants from any liability for any damage during such travel and extension activities and/or during regular, scheduled activities.

ACKNOWLEDGEMENT AND SIGNATURE

By checking the “Waiver of Liability” box during the registration process I confirm that I have read the above Waiver of Liability and as Parent/Guardian and on behalf of any other participant that I may bring to any activities of the School, I hereby accept all terms and conditions contained therein. I also confirm that checking the box is the same as providing my signature.
